Models of care for diabetes in the community (MODCOM)

We are comparing two different approaches to providing care for people with diabetes. One method is for GPs and practice nurses to work with colleagues in hospital. Another is to provide community clinics for diabetes, which act as a 'bridge' between practices and the hospital. Surgeries taking part in the study will use one of these two approaches for two years.

Our aim is to find out which approach is more effective in helping patients with diabetes achieve the best possible control.

If you agree and are eligible to take part, we will ask you to attend for two assessments at your surgery with a research nurse. The first assessment will be at the beginning of the study and the second 18 months later. At the first assessment the research nurse will explain the study in more detail, and ask you to confirm your consent to take part in writing. Taking part in the study will not affect the care you receive, or whether you are invited to attend a community clinic.
